History and Physical Exam

Volar plate injuries occur with PIP hyperextension. There is pain about the PIP joint, particularly on the volar surface. If dorsal dislocation or subluxation occurs, there will be swelling and tenderness on the dorsal aspect of the PIP joint. Stress testing of the ulnar and radial collateral ligaments of the PIP should be performed. A digital block may be needed for an adequate examination.
