ED Management
Emergent orthopedic consultation is indicated - these are unstable fractures. Treatment usually includes closed reduction and internal fixation with percutaneous Kirschner wires. This is followed by non-weight bearing casting for 12 weeks and an orthotic for up to 1 year. More recently described, a Lisfranc joint sprain considered to be stable can be treated with immobilization for 6 weeks in a below-knee walking cast.
