Pain and mechanism of injury should prompt CT imaging, which will confirm the diagnosis. This will show widening of C1 lateral masses compared to C2, increased distance of the atlantodental interval, and fractures involving the anterior and posterior arches. CT is highly sensitive at detecting fractures, though MRI is superior for detecting associated ligamentous injury.
