Pediatrics Case 8 ED Management

Management of Toddler fracture has variability, ranging from long leg posterior mold splinting/casting, short leg splinting/casting, and controlled ankle motion (CAM) boots.  Initial recommendations also may vary based on whether the fracture is visible or not radiographically. In the absence of an institutional practice pattern for guidance, the most conservative initial treatment would be to place a long leg posterior mold splint.  Patients often resume weight bearing in 4 weeks or less.  If treating presumptively after negative x-rays, refer for repeat x-rays in 10-14 days.
